COVID-19 Policy: Our waiting room is CLOSED.  We allow one adult with the patient; please, no additional children other than patient.   Please enter the building at your scheduled appointment time wearing your mask, as required by NYS DOH.

We are located at the corner of Delaware Avenue and Hodge Avenue.  Our parking lot is located off of Hodge Avenue and is free of charge.  Please pull up to the gate and dial PEDIATRIC CARDIOLOGY at the call box; you will be asked for the patient's name and one of our staff members will be happy to open the gate.

Please bring all of your medications, or an updated medication list complete with dosages and frequency, with you to your appointment.  Also, please have the patient's insurance card(s) available.

Demographics/face sheet, any scripts for testing, and the last clinic report are REQUIRED to be faxed to us from your physician's office before we can schedule your appointment.   Fax number is 716-258-3329.   Please allow 24 hours before calling our office. 

 

Some United Healthcare policies require an insurance referral.  Please contact your primary care physician to have this in place before we schedule your appointment.
